coverage for ortho treatment

Question: if ortho treatment is completed when the patient is covered by dental insurance.
AND if the claim for it is filed also when the patient is covered by dental insurance.
BUT the patient loses insurance before the insurance pays for the treatment.
Can the insurance refuse to pay for the treatment?
In my specific case, the ortho treatment (braces) for my daughter was completed in mid March, the provider sent a claim to the insurance at the beginning of April. The insurance has not paid about $700.00 for it yet. My coverage ends May 1st. The insurance is saying that they will not pay because I will not be covered.
Can they do that?
